What we collect

When you contact us through this website we collect your name, your phone number or email address, what your enquiry is about, and anything you choose to write in the message field. Please don't include health information or NDIS plan details in the form — we'll cover those together when we talk.

Why we collect it

We use your details for one purpose: to reply to your enquiry and, if you become a client or referrer, to arrange and deliver supports. We don't use your details for marketing lists, and we don't sell or share your personal information with anyone else for their own purposes.

How it's stored

Enquiries sent through this website are delivered to us by email through our website hosting and email provider. We keep enquiry records only as long as we need them to respond to you and meet our record-keeping obligations.

Service records

If you receive supports from us, we keep the records we need to deliver safe, accountable services and to meet our obligations under the NDIS Code of Conduct and the Privacy Act 1988 (Cth). Access to those records is limited to the people who need it to support you.

Access and correction

You can ask us at any time what personal information we hold about you, and ask us to correct it if it's wrong. Email admin@cornerstonepathwaysgroup.com.au and we'll respond within a reasonable time.

Concerns

If you're unhappy with how we've handled your information, contact us first and we'll do our best to put it right. You can also contact the Office of the Australian Information Commissioner at oaic.gov.au.
